The clearer you are on this "Plan" the better, but even a vague plan, if attainable and believable to the only person that matters is okay. (Incidentally, that's you again.)

Finally, the critical step is that you actually decide to do it. This is certainly a decision - not a wish.

The resolution once made is delicate, like a newborn infant and needs nurture and care. In the beginning your own will power and commitment may be enough to sustain it and get you off to a start.

Your initial achievements can serve to encourage you to continue, but you have to see that success. And that means you need to monitor the progress. If you cannot measure it, it will be difficult to use that success to sustain the enthusiasm and commitment.
